Creating an Informed by MCG assessment and populating the patient care plan

Verify that you can schedule and conduct an Informed by MCG assessment and have the results of the assessment populate the patient's care plan with problems, interventions, and tasks.

Before you begin: Enable the CMSysAdminCCIDemo operator account.
  1. Log in to Pega Care Management by entering the CMSysAdminCCIDemo operator and the password that you specified.
  2. In the portal header, click Patients, and then enter a name in the Search for patient field.
  3. In the Patient ID column, click the ID.
  4. In the Patient care list, select a program, and then at the right of this section, click ModifyAdd to programTask.
  5. In the Select category field of the Add task dialog box, select Schedule MCG Assessment.
  6. In the Search task field, enter End of life care.
  7. In the Search results list, click the task name, complete the fields for the task schedule, and click Submit.
  8. In the To do list of the patient profile, click Begin at the right of the end of life care task.
  9. In the Member details section, click Submit.
  10. In the Select due date field, click Now, complete all fields in each step of the wizard, and then click Submit.
  11. When the How is your caregiver doing with your care? question is displayed, click the Doesn't meet needs response.
  12. Complete the fields and click Continue.
  13. Click Finish when you complete the assessment.
    Result: The results of the assessment automatically populates the patient care plan with problems, barriers, and tasks.
  14. Check the items for the patient program and click Submit.
    Result: The system creates the tasks. If you are unable to get the results when performing this task, contact your MCG account representative.
