Details tab
The details tab allows users to enter the primary care physician (PCP) information.
Pega Foundation for Healthcare
Users can search by the NPI or first name of the primary care physician and then select the PCP. The name and speciality of the PCP would be auto populated.

NPI | This fields allows user to choose a Primary Care physician NPI or Name. | No |
Effective Date | Effective date is the member’s effective date in association with the PCP. | Conditional |
End Date | End date is the member’s end date in association with the PCP. | Conditional |
The details tab also allows users to capture Medicare and Medicaid details about the member.

Medicare Number | Member’s Medicare beneficiary number. (MBI) | Conditional |
Eligibility / qualifying reason | If user checks ‘Medicare’, system allows the users to capture the eligibility reason. Eligibility reason such as member’s turning to age 65 or End Stage Renal Disease (ESRD) can be captured here. | Conditional |
Qualifying date | Member’s Medicare qualifying date. | Conditional |
Medicare plan type | Medicare plan type (Part A, Part B, and so on.) | No |
Medicare Number | Member’s Medicare or HICN number. | No |
Effective date | Plan type effective and end date. | No |
The details tab allows users to capture members bank account information.
