Care plan

To support best practices in developing Care Management and Disease Management plans, you can create standard care plan items and publish them for use by the care managers to build the member-specific plans in collaboration with the member. Build the care plan items in a specific order because of the inheritance hierarchy. For example, create a task before an intervention or goal because the interventions and goals reuse the tasks that were already created.

Pega Care Management includes assessment and care plan templates which care teams can easily customize to meet their needs and accommodate their unique workflows. Care plans can include Pega’s best-in-class automated processing to improve consistency, accuracy, and productivity. You can easily configure workflow activities to invite patients to participate and maintain on-going multi-channel engagements with them, including specific goals and tasks for every participant. This allows care teams to establish best practice workflows, schedule, and tasks to optimize the patient care.

In the care planning stage, Pega Care Management is presented with the list of system-identified care plan items (based on answers in the assessments) and the list of care plan items configured for the program in the Business Analyst portal. From the identified care plan items list, the care manager can select relevant items and submit them to proceed to program enrollment. The system creates a new care plan along with the embedded tasks for the patient. All the information populates the Care tab on the Patient 360. Additionally, you can view the work items that are associated with the care plan by selecting the appropriate program type from the drop-down menu on the Care tab.

Data model

In App Studio, you can view the data model for Care Plan in Case Types.

Click the Data model tab on the Care Plan page.

Changing the number of tasks displayed on a care plan

You can change the number of tasks that are displayed on a care plan, for example, tasks displayed for the past month or tasks displayed for the next month. By default, the Pega Care Manager application displays tasks for the past seven days.

  1. In the header of Dev Studio, enter pyLoadMyCasesNestedOneWP and press Enter.
  2. Open the report definition in the PegaHC-Care-Work class.
  3. Click Save as and save the change to your implementation-layer ruleset.
  4. On the Query tab, scroll to the Edit filters section.
  5. In row E (Column Source:.CMTask.StartDateTime), click Select values.
  6. In the Select Values list, scroll to the Day section and select an option.
    For example: You might select Next 30 Days.
  7. Click Submit.
  8. Click Save, and then click Check in.
